SWITCH KIT CHECKLIST – Identify any companies you have authorized to automatically withdraw funds from your accounts.
ACCOUNT CLOSURE FORM – Close your accounts elsewhere by mailing this completed form to previous financial institution(s).
DIRECT DEPOSIT CHANGE FORM – Update your direct deposit to be sent to your Bluestone FCU account.
AUTOMATIC WITHDRAWAL CHANGE FORM – Update automatic withdrawals to your Bluestone FCU account by sending each company making withdrawals this completed form.
